Following are documents required in general.
· Addresses for the last 5 years including dates of residence at each;
· Last address abroad for more than one year and dates of residence;
· Parents' names (maiden name for mother), date and place of birth and place of current residence;
· Copies of status documents for entire stay in the US (I-797s, I-20s, EADs);
· Name, type and dates involved in any organizations (such as professional organizations);
· Six identical photographs (see attached photo specification sheet);
· Copies of all passport pages (including blank pages) and color copies of identity and visa pages;
· Copies of family status documents (birth certificates, marriage certificate, and divorce judgments, if any); and
· Sealed medical exam. See information on civil surgeons at: https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV). The doctor who performs the exam will have the current From I-693 which will need to be completed.
For medical I would recommend you to find a doctor from above link. It is better to go to non asian doctor (most of them prefer to do complete health checkups and vaccines at their own clinic that could be from $500-$800). First contact one of them and ask what are the requirements to get a medical report for immigration. They provide you list of vaccines and other checkups they need to do. Such doctors do not accept insurance for medical exam for USCIS , ask them if s/he accept medical records from your personal doctor. Most of non Indian doctors accepts records or medical exam from your personal doctor. If your civil surgeon accepts personal doctor's exam contact your doctor and explain him that you need health report for USCIS and need vaccines, medical exams. Since most of us already have most of those required vaccines but have no record, doctor schedule your titer test. Based on your antibodies level and presence doctor would give you all required shots or booster shots and are covered by insurance. It is a quiet time consuming ( since you have to visit personal doctor couple of times) but it worth.
