Last year i applied for TPS registration only not for EAD and this time also i am renewing TPS registration and do not applying for EAD because i Have H1B right now. I am on H1B now and i would like to renew my TPS now. Before H1B approved i did also apply for OPT extension and it was also approved after my H1B approved. While filing the form I-765 i am confusing on Number 11.I know i have to select Yes but which date and address should i have to provide OPT, OPT extension or H1B file date and address? In number 15. I should write H1B immigration status right? I am not sure. Also, I am not sure what are the documents i should have to send to USCIS. Most of them i know are listed below-

-I765
-I821
-Password Copy
-Previous TPS approval Copy(Front and Back)
-I94 (front and back)
-2 PP photos
-Filing fees $465
-proof of address (If we moved to new address)

Please response me if anyone knows more about this. It will be really help.

On i-765 number 11: Provide the info of the OPT extension (which is your latest employment authorization application not H1B)
number 15: Yes you are on H1B.
All you need to send is i-821 and i-765 with correctly checked boxes for reregistration. No other documents required for reregistration.
Fee you need to send is $85 for biometrics. No filing fee for reregistration and no fee for i-765 since you are not applying for TPS EAD.

ny2- just a silly question---
If you are also applying for a renewal of EAD, on form I-765 would you check the box that says [permission to accept employment] or [Renewal of my permission to accept employment] ? And if you check the later one does that mean we need to also attach our previous form I-765 or just the EAD card photocopy.
