Before the hospital sends to collection agency, negotiate (with hospital). Tell them that you will pay but are able to pay only $10, $20, (how much you can) per month. I have done that in the past. I told them that I can pay only $25 per month and they agreed and now I have paid in full.
Alternatively, you can ask them if you can apply for financial assistance. I am sure hospitals have financial aid. I recently did that and they approved. Out of around $8000 to $9000, now I have to pay only $300!

For both of the above situations, they probably will ask for your bank balance, and income.
Don't worry, you will be fine, but don't delay in negotiating with the hospital because once it goes to collection agency, then it will be out of hand.

How about the doctor feee spartyyyyyyyyyyyyyy

Try to talk to the social worker and you may be waived or credited depending up on your financial status. To the doctors bill keep sending 5-10 that will stop going to collection. Send them a request letter for waiving the fee and services with thank you note for their services. There is nothing to loose.
The other day I asked a guy , who was selling car for 700, can you give me free and I will ride  fixing it. He or she might change mind  and what the heck, may say come and get it or may not respond. Nothing to loose.

You mean co-pay? Didn't you pay that during the visit?
The hospital's bill includes doctor's fee.
If you mean by doctor's fee in clinic, then you can negotiate with them as well.
